As I began preparing my Institute for Healthcare Improvement Forum (IHI) presentation this year, I wanted to share other stories that would have an impact involving children so I’ve added this story with resources available for participants attending our session.

One of the worst cases of unsafe injections this year (April) came from Colorado where the parents of 450 children were told their kids might be exposed to HIV after a medical assistant reused syringes giving them flu shots.
